Mostly Spear is just... confusing to King. WHY WOULD YOU HEAL ME AND WANT TO TAKE ALL THE PAYMENT FOR IT????? IF IT WAS GILGAMESH he'd probably take it with a level of "oh of course you would, I'm the king" and just leave it at that, but King took it instead as... Spear thinks he needs to coddle / baby him. So he feels like he needs to one-up Spear, in a way.
And reviving him after Cassim Cassim'd Spear was his own way of doing that, even if he was upset with Spear refusing to tell him who did it at first! And he kind of felt like he ... one-upped him in a way he had sex with him EVEN IF HE WAS DRUNK, because he draped all over him in front of Cassim and ha take that this is mine B)
Overall it's very... confusing dere feelings hidden under piles and piles of denial and unhealthy possessiveness.

And he couldn't just. Leave King to die. So taking responsibility to help this crazy thing bleeding out on the ground became TAKE RESPONSIBILITY FOR THE CRAZY THING and just. grow fond of it.
Intellectually he does get that King is dangerous and insane and probably not someone he should associate with but WHOOPS HE CARES? TOO MUCH? TO LET GO?
basically it is straight up dere and a lot of worry and yes some possessiveness like King is his pet murderer or something BUT STILL SEEING HIM AS A. PERSON. AND SOMEONE HE SUPER CARES ABOUT. EVEN THOUGH HE KNOWS HE SHOULDN'T. and relies on him a bit as well which is possibly the worst part.
